DHS Releases Report on Little Girl's Death (Aug 11, 2011)

OKLAHOMA CITY (AP) — An Oklahoma Department of Human Services report on the death of a 5-year-old girl says child welfare workers failed to follow the agency's policies and procedures.

The agency released a report in the case of Serenity Deal, who died June 4 while in the custody of her father, Sean Brooks.

A previously released report by the Oklahoma Commission on Children and Youth indicated DHS workers recommended Serenity live with Brooks despite reports that she returned from visits with him with black eyes and bruises.

DHS spokeswoman Sheree Powell told The Oklahoman the report shows disagreement existed as to how quickly Serenity should've been reunited with Brooks, but transcripts indicated that all parties expressed to the court that reunification was appropriate.

Brooks has been charged with first-degree murder in Serenity's death.
